We are excited that you will be joining us soon. Below are printable forms that need to be completed by each participant before your adventure begins.
One per participant, for minors and adults.
To be completed by parent or guardian.
To be completed by each adult.
To be completed by anyone staying at Camp Travis without programming.
If an adult is participating:
Each adult will turn in an agreement to participate form as well as the medical profile for adults.
If a minor is participating:
Each minor will turn in an agreement to participate form as well as the medical profile for minors, both of which have been completed by a parent or guardian.
